Isolated vagus nerve palsy,or vagal mononeuritis,is a distinct clinical entity. This disorder usually presents as vocal cord paralysis caused by recurrent laryngeal nerve palsy. Most cases of acquired recurrent nerve palsy have an identifiable cause, the most common being trauma(including surgery),malignancy, mechanical compression, and various central nervous system disorders. However,a substantial proportion of cases are idiopathic. Viruses including herpes simplex virus(HSV)have been suspected, but not proven,to cause vagal mononeuritis. We report a case of HSV-induced vagal neuritis that resulted in protracted unilateral vocal cord paralysis despite therapy with acyclovir. |
